Need an NDIS-compliant quote before placing your order? Use the form below and we’ll prepare a formal quote you can send to your plan manager or the NDIA for funding approval.

This form is for customers who have not yet purchased and would like to confirm eligibility or secure reimbursement approval first.

If you've already placed an order, please use our NDIS Invoice Request Form instead.

Quotes are valid for 30 days from the date of issue.

Some items may not be eligible for NDIS reimbursement — check with your plan manager.

Once approved, you’ll be able to place your order using the quote number.
